Post-Assisted Living Program
Patient Prognosis
As an assisted living community’s population ages, individual residents might develop greater dependency for transferring, bowel and bladder incontinence management, or eating. Some residents might develop symptoms of dementia that exceed the staffing capabilities of the assisted living community.
Some residents might experience acute illnesses that require hospitalization and follow-up skilled nursing or rehabilitation. Other residents may have depleted the financial resources that have allowed them to stay in an assisted living community. In all of these scenarios, The Laurels’ Post-Assisted Living Program can provide a solution.
Program Design
The Laurels’ licensed nursing facilities provide both short-term medically complex services and quality extended stay programs. Our facilities are Medicare and Medicaid certified and have contracts with a number of commercial health insurers.
If a resident can no longer be cared for in an assisted living environment, The Laurels can provide a smooth and compassionate transition to a nearby licensed nursing facility. The transition will include quick processing of inquiry information and transporting the resident for a facility tour. Should the resident and family select a Laurel facility, we can provide moving and other transition services.
