The Laurels provides a safe and comfortable environment while helping you achieve the highest level of physical, social and emotional independence. To assist you in this transition, this web page will explain what to expect during your stay.

 

Communication is Key

 

Because we are your partners in caring, we want to understand how we can best serve you. This starts with a care conference with you, your family and our team of professionals. We encourage you and your family to ask questions and make suggestions regarding your stay at The Laurels.

 

The Care Plan

 

To ensure the best care and consistent service, we will establish a personal ”care plan“ for your daily medical and personal needs. This care plan details what medications, therapy or any special treatments you may need and when they occur. The care plan also describes the degree of assistance you may need in daily dining, bathing, grooming and other personal care needs. As you become more independent, your care plan may be modified to better suit your personal care needs.

 

What to Bring

 

We will need copies of the following upon your admission:

  • Medicare Card

  • Insurance Card

  • Medicaid Card

  • Social Security Card

  • Durable Power of Attorney/Guardianship Document

  • Living Will

We recommend that you bring a 5-7 day supply of comfortable, seasonally appropriate clothing marked with your name as well as any personal toiletries you use. Please bring personal belongings such as pictures, photo albums, writing supplies, knick-knacks, pillows, throws…anything that will help you feel at home. You may also have a phone installed in your room for your private use. We can assist you in making arrangements with the local phone company.

 

Visiting with Friends & Family

 

Whether you are here for rehabilitation or long-term caring, invite your friends and family to visit as often as possible. We have special areas inside and outdoors that will make your visits private and enjoyable. Invite your family and friends to participate in recreation programs with you. Suggestions & Concerns

 

The Laurels is your partner in caring and we want to make sure that everything that occurs with your care plan is comfortable for you. If you have questions or concerns regarding any aspect of your care or service, speak to the Associate within that department or ask to speak to the Social Worker. If, for some reason, you or your family have a concern that was not thoroughly addressed by our Associate, inform The Laurels’ Administrator and/or Director of Nursing. We are available and want to know your concerns. There are suggestion forms available throughout the facility should you choose to express any concerns in writing.

 

Our Admissions Handbook

 

Upon admission, you will receive an Admissions Handbook. The Admissions Handbook is provided to answer questions regarding visiting hours, clothing, accommodations, furnishings, mail, salon services, payment and many other important aspects of living at The Laurels.
